No. Your practice decides whether each case is appropriate. A referral simply means the applicant may be a potential fit for evaluation. It does not create an obligation to accept or treat the patient.

Who owns the treatment plan?

The treating doctor owns the treatment plan. Smile Fund USA does not make clinical recommendations or interfere with diagnosis, sequencing, materials, fees, or case acceptance.

What does the grant cover?

Grant support is applied toward eligible treatment according to Smile Fund USA program guidelines. The exact amount and scope may vary by patient, funding availability, and case approval.

Who pays the practice?

Approved grant support is applied toward eligible treatment costs according to program guidelines. Any remaining patient responsibility, financing, or balance is handled through the practice’s normal financial process before treatment begins.

Is this free dentistry?

No. Smile Fund USA is not asking providers to deliver unlimited free care. The program is designed to help reduce the affordability gap for qualified applicants while allowing providers to maintain normal clinical and financial standards.

Can my practice decline a case?

Yes. If a case is not clinically appropriate, not financially feasible, outside your scope, or not a good fit for your office, your practice may decline the case.

Does Smile Fund USA require a donation from providers?

No. Provider participation is not based on making a donation. Any charitable contribution, if offered, is separate and optional.

Smile Fund USA is interested in implant-focused general dentists, oral surgeons, periodontists, prosthodontists, full-arch teams, and restorative practices with experience managing implant-related cases.
