This is not a request for free dentistry. It is a structured provider partnership program built around patient access, clinical autonomy, and responsible grant support.

A Selective Partnership Model for Implant-Focused Providers

Smile Fund USA works with qualified dental professionals who understand the life-changing impact of implant and restorative care.

Missing or failing teeth can affect far more than appearance. For many patients, treatment can restore function, confidence, dignity, and quality of life. However, even motivated patients often delay or abandon care because of cost.

Our role is to help bridge that affordability gap for qualified applicants while partnering with providers who are equipped to evaluate and treat cases appropriately.

Provider participation is selective. Smile Fund USA is currently focused on building a limited founding provider network, beginning in California with a current funded opportunity in San Diego County.

How the Partnership Works

Smile Fund USA is designed to support access to care without interfering with the doctor-patient relationship or the treatment planning process.

1. Smile Fund USA Screens Applications

Patients apply through Smile Fund USA and provide information about their dental needs, location, financial circumstances, and treatment goals. Smile Fund USA reviews applications for baseline eligibility and potential program fit before referring appropriate cases to a provider.

2. Appropriate Cases Are Referred to a Partner Practice

When an applicant appears to be a potential fit, Smile Fund USA may refer the patient to a participating provider for evaluation. Referral does not guarantee treatment, approval, or case acceptance.

3. The Practice Determines Clinical Fit

The treating doctor and dental team remain fully responsible for the clinical evaluation. Your practice decides whether the case is appropriate based on diagnosis, patient health, treatment complexity, scheduling capacity, restorative needs, and your own professional judgment.

4. Grant Support Is Applied According to Program Guidelines

When a patient is approved for grant support, the approved grant amount is applied toward eligible treatment according to Smile Fund USA program guidelines. Grant support is intended to reduce the patient’s financial barrier. It is not unlimited funding, and it does not replace your office’s normal treatment planning, financial consent, or payment process.

5. The Patient Proceeds Only If the Case Remains Appropriate

Treatment moves forward only when the case remains clinically appropriate, financially feasible, and accepted by both the patient and the treating practice. All clinical decisions remain with the treating doctor.

Smile Fund USA is designed to be clear, structured, and respectful of the provider’s role.

This partnership is not:

  • A requirement to provide unlimited free dentistry

  • A replacement for your office’s normal consultation or treatment planning process
